Commendation Form

  This form is to be used for members or the public to file a complaint against a member of the Altoona Police Department. This form should not be used to contest a citation or arrest. That should be done in a court of law. Below are the instructions for use of this form:

1) Obtain a form from the Police Department lobby or utilize the online version on our website.

2) Complete all fields you are able to. We ask that you provide your name to fully investigate your complaint but anonymous complaints will be accepted. 

3) Submit the form:

a. Via the website

b. By email to APDcommandstaff@altoona-wi.gov

c. In Person at the Altoona Police Department (1904 Spooner Avenue, Altoona, WI)

d. By Mail to Altoona Police, 1904 Spooner Ave, Altoona, WI 54720. 

4) When we received your complaint we will provide you with a receipt of the complaint and provide you an update when your complaint has been investigated.

If you do not wish to speak to a certain employee, please ask for a separate employee to assist you.

 Yes, Wisconsin State Statute 946.66 states that filing a false report of police misconduct with a law enforcement agency is a criminal offense. This state statute is not meant to inhibit the reporting of legitimate complaints — it’s for the discouragement of intentionally bringing false complaints against law enforcement personnel. This is NOT a forum to challenge a citation or arrest. That must be done in a court-of-law utilizing the court date you were given at the time of the arrest or citation
